  • Health Disparities Collaborative Website

    The Bureau of Primary Health Care (BPHC) created the Health Disparities Collaborative (HDC) website to facilitate the sharing of information related to the HDC initiative.
  • Also hosted on the HDC Website are all of the Virtual Offices for the various clusters and pilots sponsored by the BPHC. The Southeast Atlantic Cluster's Virtual Office resides there, where teams can get information on cluster conference calls, download training materials, view archived presentations, and even connect up to CIS Training Webcasts.

HDC Reporting

  • Health Disparities National Reporting (HDNR) Website

    As a health center participating in the Health Disparities Collaborative (HDC) you are required to report your clinical measure data to the Bureau of Primary Health Care (BPHC) on a monthly basis. The BPHC created the Health Disparities National Reporting (HDNR) website to enable you to report your clinical measure data electronically.

SAC Key Measures Dashboard

  • SAC Key Measure Dashboard Website

    The SAC Key Measure Dashboard is a web-based, interactive data visualization tool designed to display health center key measure data in an easily accessible way.
  • The dashboard provides a single page "at a glance" high level view of your health center's performance and the ability to "drill-down" into the data for a more in-depth review.
  • The dashboard also allows each health center to compare their performance against other centers in their state, the cluster, and the national average!

HDC Help Desk

  • National Health Disparities Collaborative Help Desk

    Part of the HDC Website, the National HDC Help Desk is the submission and tracking tool used by all the HDC Clusters to work with Health Centers on CIS related issues. All support needs of the Health Centers must be logged in the National HDC Help Desk before an IS Specialist can provide assistance.
  • For assistance with getting started, please review the Help Desk Overview presentation. If you need assistance in learning to use the Help Desk you can also schedule a training session with an IS Specialist, just click here.
  • CIS Knowledge Base

    The National HDC Help Desk is also the home of the Clinical Information Systems Knowledge Base. The Knowledge Base holds a variety of answers to many of the commonly asked questions in regards to PECS, Reporting, the different HDC Websites, and more. It's constantly growing to include even more information that will be helpful in your support process. We encourage everyone to look through the Knowledge Base before opening a ticket.

  • Texas Association of Community Health Centers (TACHC) Online PECS Training Website

    Funded by the Bureau of Primary Health Care (BPHC), the Texas Association of Community Health Centers (TACHC) created an online training resource for participation in the Health Disparities Collaboratives.� This training includes: An Overview, Chronic Care, PECS Training, Asthma, Depression, and Diabetes.
